ON-SITE TESTING & AFTER HOUR CALL OUT
We are committed to providing the highest quality of health care services at the most competitive rates. These On-Site services consist of Drug Testing, EBT, PFT, Fit Testing and Physical Exams conveniently performed at your facility. These services will be billed directly to your company and we expect payment in full directly from your company.
(Do not turn into your Insurance Company unless they will pay 100 % of the bill.)

*Mileage $0.60 / per mile
You will be billed $0.60 for each mile that our personnel travels to and from your designated site.

*Hourly Rate $25 / per hour / per personnel
Monday - Friday
8:00am - 4:00pm
You will be billed hourly for each personnel hour minimum) for each hour each personnel spends at your designated site including travel time.
(Dr. Craig $125 / per hour / onsite)

$ 50 / every hour after
You will be billed $100 for the first hour each personnel is called out after office hours and $50 for each hour after.

Dr. Craig is also available to meet you (after hours) to treat your injured employees at your companies preferred hospital or at our facility. Please call the answering service.
